A washer box looks like a small thing: a recessed box with two valves and a standpipe. It is also the single most common source of a flooded laundry room, because a burst supply hose or a standpipe that is too short empties into the house rather than the drain.
Bathrooms are the same story one room over: the tub, the valve behind it and the drain under it are set once, and the water test before the wall closes is what proves they are right.

The end of a tub job: tile on, matte black valve trim and spout fitted, tub in service. Behind that trim is a valve set to the finished wall thickness and a tub that was filled and watched before anything was closed.

Two things, in this order: a burst rubber supply hose, and a standpipe or trap that cannot take the machine's discharge rate. A proper washer box with quarter-turn valves and braided hoses removes the first; a correctly sized standpipe removes the second.
Usually yes, if the drain can be re-routed to a vented line. That is a wall-open job like the one photographed here, and it is worth doing once rather than working around a bad position for years.
